Place chicken breasts in a single layer at the bottom of the slow cooker for even cooking.

Add the thinly sliced onion and bell peppers over the chicken, followed by minced garlic on top.

Pour chicken broth and Worcestershire sauce over the ingredients. Sprinkle with Italian seasoning, salt, and pepper.

Cover and cook on low for 6-7 hours or on high for 3-4 hours, until chicken is tender and easily shreds.

Remove chicken from slow cooker, shred with two forks, then return to the cooker. Stir with vegetables and broth to combine flavors.

Approximately 10 minutes before serving, place provolone cheese slices on top of the chicken mixture. Cover and allow to melt.

Slice hoagie rolls in half and fill each with the chicken and melted cheese mixture.
